The script for this page didn't run
Everything you see above is styling only. The name search, entry, and drawing all need this page's JavaScript, and it was either removed when the page was saved or blocked by the site's security settings.
The fix is to upload this file to the district file manager and embed it in an iframe, rather than pasting the code into the page editor. An iframe runs the file as its own document, so nothing gets stripped.

Also switch off Accepting responses in the Google Form — that is what actually stops submissions. This toggle only updates what students see here. Nothing already entered is affected, and it reopens by itself tomorrow.
Open the responses sheet, select the columns with the timestamp, email, and name, copy, and paste below.
Staff sign-in is managed in the Firebase console (Authentication → Users), not here.
